 2025 Spotlight Project

Mercy Health Foundation - $25,000

In 2025, The Moniker Foundation supported a 12-month initiative aimed at aligning youth behavioral health services across community partners in rural, Southwest Colorado. This initiative was born out of the lack of access to mental health services in the region, which was identified as the top need from the Mercy Community Health Needs Assessment, as well as Axis Health System's Gap Analysis. Additionally, services for youth and their families are often siloed, and gaps exist along the continuum of care. The intent of the project was to convene healthcare providers, social services, schools, law-enforcement and public agencies, and community-based nonprofits to co-design and pilot a coordinated care approach that improves access, equity, and outcomes for youth with behavioral health needs.  Accordingly, the Mercy Health Foundation requested leadership seed funding from The Moniker Foundation to begin convening partners working in the space of youth mental health in Southwest Colorado. The funds would benefit youth and their families directly, but also dozens of nonprofits and three hospital/ health systems in the region, through increased collaboration and efficiency. Ultimately, this initiative will help establish a community-centered care model, linking services and support for youth and their families.

  • Manna - $10,000 multi-year operating support grant
  • Team Up -  $7,000 for Summer Program Scholarships
